Table of
Correspondences
Instructions: If the District Technology
plan does not follow the sequence of the required elements for the technology
plan, the district is requested to submit a "Table of Correspondences",
of which the following is a sample. In
this table, for each required element, where the element is to be found within
the existing technology plan should be specified in the "location"
column. Use page numbers, paragraph
numbers, section labels, and/or leading words for relevant text to point
precisely to the matching components in the district plan. Post the Table of Correspondences (and
Addendum if included) on the web and include the exact URL in the district's
Consolidated Application.

Technology Plan Addendum
Directions: If the technology plan does
not currently address all the required elements, the district should file an
addendum that effectively certifies that the required elements will, in fact,
be incorporated into the next revision of the plan. The purpose of the Addendum is to add the required elements which
have not yet been incorporated into the plan.
NOTE: It is anticipated that when
the district files its next Consolidated Application, the technology plan will
be expected to include all the required elements. It is preferred that the technology plan follow the sequence of
required elements, with any additional components required for e-rate purposes
added at the end. If the plan does
follow the sequence, then the table of correspondences will no longer be
needed.
To
bring the current plan into compliance for the purposes of the Consolidated
Application, the revisions may be in the form of an Addendum to the existing
technology plan. However, given the
brief amount of time available in which to submit the upcoming application for 2002-3 funds, it may not be
feasible to obtain necessary board approval for an Addendum in time.
Therefore,
to meet the requirements for the June 2002 application, the Addendum may be
affixed to the Table of Correspondences.

The Addendum included at
the end of the Table of Correspondences should include: 1.
Statement
that the existing plan does not currently address certain required
items. (Specify which items [1-17]). 2.
The
addition of each of the required elements (to the extent possible within the
current time constraints) 3.
Statement
that work will be undertaken in the immediate future to incorporate the
required elements into the technology plan. 4.
The
date the plan revision will be completed. 5.
Statement
that certifies that, upon completion of the plan revision, the district will
both a.
Post
the revision on the district web site (URL specified), and b.
Notify
the state that the revision is complete and ready for review AND 6.
Statement
(if applicable) that a technology plan revision is currently in progress. 7.
Statement
(if applicable) that although not formally a part of the technology plan,
certain elements (specify which) are, in fact, informally in place. In this case, identify each element and
provide a brief description of how the element is, in fact, in effect. 8.
Statement
(if applicable) that although the current technology plan is not yet due for
revision, work will be undertaken during the forthcoming academic year to
bring the existing plan into compliance with the required elements. |
